Register as New Patient
The Hampstead Group Practice welcomes all new patients and is committed to providing an equitable service to all.
Before registering please check that you are within our practice boundary by clicking on our postcode checker below. If your postcode falls within the inner boundary we can register you as a patient. If your postcode falls within the outer boundary please complete the “Out of Area” registration form.
To register, everyone will need to complete Forms 1 and 2. Form 3 (Out of Area Registration) only needs to be completed if your postcode falls within the outer boundary in the postcode checker. Each family member must register individually:
1. GMS1 form (for both adults and children)
2. New patient questionnaire (Adult or Child)
3. Out of Area registration form
